The imprint order form is available for download by using our IMPRINT FORM. Use the PEW EDITION IMPRINT FORM for imprinting your Evangelical Lutheran Worship pew edition. Please send this completed form to Augsburg Fortress Customer Service either via fax (519) 748-9835 or postal service (500 Trillium Drive, P.O. Box 9940, Kitchener, ON N2G 4Y4). Please note that the standard delivery time is 3-4 weeks. To specify a particular date by which you need this product, please elaborate in the "special request" box of the order form.
